Waiver Agreements *

A Waiver is required by all individuals participating in the Holiday Parade.  It is the responsibility of the entrant primary contact to collect and submit waivers with signature from all entry participants, including walkers, riders, adults and youth.  Parental/guardian signature is required for all participants under the age of 18. Please scan and upload all agreements as one file.  If you need additional time to collect more signatures, please email the remaining to hello@woodlandchamber.org prior to November 18, 2022.
